Your Responsabilities
To ensure efficient billing and payment processing, we kindly ask that you review and acknowledge the following policies:
Insurance & Identification: You agree to provide a valid insurance card and a photo ID at each visit. This information is necessary for us to submit medical claims on your behalf.
Payment at Time of Service: Co-payments and co-insurance amounts are due at the time of your appointment. We accept cash, personal checks, and Visa/MasterCard debit or credit cards.
Fees for Returned Checks & Late Payments: If a check is returned due to insufficient funds, a $30 fee will be applied. Additionally, any co-payments not made at the time of service may incur a $30 late fee.
Insurance Claims & Your Responsibility:While we assist in submitting insurance claims, it is ultimately your responsibility to ensure they are processed and paid. If needed, you may need to follow up with your insurance provider or employer to resolve any issues.
Out-of-Pocket Payments: Since your insurance policy is an agreement between you and your provider, you may be required to pay for services upfront while you work with your insurance company for reimbursement.
